How can I volunteer to help at the Clinic?
Monetary donations are essential for the operation of the Clinic. All donations are tax deductible.
Gwinnett Community Clinic is a volunteer operated clinic, so physicians, nurses, and other clinical practitioners are needed to work during the first four weeks of each month on Tuesday and Thursday evenings from 6 pm til 9 pm or so doing patient assessments, drawing blood, taking histories, vital signs and so forth.
Clerical volunteers are needed for patient check in on clinic nights (see above) and for working in the Clinic office during regular clinic business operating hours. (Monday- Thursday, from 9 am until 2 pm.)
All volunteers should contact the Executive Director for more information and to schedule a time for orientation.
I am a physician or nurse. How can I help?
Gwinnett Community Clinic is a primary care clinic and needs physicians, nurse practitioners and physicians assistants to assess, diagnose and prescribe a plan of care for each patient.
Nurses prepare patients to be seen by these providers by taking vital signs, doing brief history, recording medications, chief medical complaints and reason for the visit for each patient. Nurses may also draw blood, do an electrocardiogram, measure INR, and assist with discharge planning for each patient.

I would like to receive medical care and services from Gwinnett Community Clinic. How do I become a patient?
Gwinnett Community Clinic only treats uninsured Gwinnett County residents who qualify with our financial guidelines. All prospective patients must have a qualifying appointment.
Patients who wish to receive services from Gwinnett Community Clinic (GCC) must:
• Meet household income qualifying requirements
• Be a Gwinnett County resident
• Have no insurance, including Medicare or Medicaid. Patients
• Must call for a qualification appointment at 770.985.1199, extension 6
Patients who have a qualification appointment MUST bring the following documents:
• Photo ID - Georgia drivers license preferred
• Utility Bill - with your name and address (This is necessary to establish residency of Gwinnett County)
• Household Income - We do not use individual income. We use all income and count all people at
the residence. Provide current 1040 (Federal tax returns) and current pay stubs.
• Annual Updates - are necessary to maintain eligibility
I am not a Gwinnett County resident. Can I receive care?
Gwinnett Community Clinic only treats uninsured, indigent residents of Gwinnett County.
What are the Clinic hours?
The Clinic operates by APPOINTMENT ONLY.
Qualifying appointments are Monday through Thursday, 9 am until 2 pm. Clinic medical appointments are the first four weeks of each month on Tuesday and Thursday evening at 6 pm and 6:30 pm. The Clinic is closed every fifth week and follows the Gwinnett County Public Schools holiday and inclement weather schedule.

Where is the Clinic located?
Gwinnett Community Clinic is located in the Eastside Heritage Center at 2160 Fountain Drive in Snellville, 30078. The Clinic office where qualification appointments are held is off the main lobby, after entering through the Heritage Center visitor entrance. Clinical medical patients are treated in the Eastside Wound Clinic. Enter the Heritage Center through the Wound Clinic entrance adjacent to the visitor entrance.
