Patient Qualifications

Patients who wish to receive services from Gwinnett Community Clinic must:
           • Meet household income qualifying requirements
           • Be a Gwinnett County resident
           • Have no insurance, including Medicare or Medicaid.
           • Patients must call for a qualification appointment at 770.985.1199 extension 6

Patients who have a qualification appointment MUST bring the following documents:
           • Photo ID- Georgia drivers license preferred
           • Utility Bill with your name and address (This is necessary to establish
              residency of Gwinnett County)
           • Household Income- We do not use individual income. We use all income and count
              all people at the residence. Provide current 1040 (Federal tax returns) and current pay stubs.
           • Annual Updates are necessary to maintain eligibility
